Application for Issuance of Certificate for students

The following certificates can be obtained from the Certificate Issuing Machine (ACM) at various locations on campus.

-Transcript【JPN or ENG】
-Certificate of enrollment【JPN or ENG】
-Certificate of expected graduation/completion【JPN or ENG】
    *Only students in the relevant year of graduation/completion (4th year undergraduates/2nd year             master’s students) can be issued.
-Student discount coupon for JR【JPN】
-Health certificate【JPN】

In detail is the following.  各種証明書

For other certificates, please fill out the application form at student affairs section of School of Agriculture, Graduate school of Agriculture.
 (Weekdays from 9:45 a.m. to 3:30 p.m. except during the year-end and New Year holidays)
Other certificates cannot be issued on the same day. Please apply for other certificates well in advance.

Application for Issuance of Certificate for Graduates

Those who have graduated from the School of Agriculture or who have completed the Graduate School of Agricultural and wish to apply for the issuance of certificates of gradation/completion, or their academic transcript, are asked to complete the following procedures.

*We do not accept applications by phone, fax, or email.
*We do not issue them on the same day. Please apply well in advance.
*Certificates will be issued as soon as required documents are prepared.

1.Application procedure

If you wish for certificates to be delivered, send the following documents to us by post. (Only for Domestic)

Please make sure to apply well in advance, considering factors such as postal conditions.

Please mail the following three items to us.

  1. Application Form for issuance of Certificates
    *Please download the form from this page.
  2. The photocopy of your residence card, passport etc.
  3. Stamped, self-addressed envelope (with your postal code, name and address, a stamp)

Application at the academic affairs section

*Hours: 8:30 to 17:00 weekdays; closed Saturdays, Sundays, national and New Year’s holidays.
Please not that issuance services may be temporarily unavailable.

Please submit us the following two items.

  2. The photocopy of your residence card, passport etc.

Certificate cannot be issued on the day the request is submitted.
You can pick them up at the academic affairs counter or receive them by post.
If you wish for them to be delivered, bring stamped, self-addressed envelope when you apply them.

Applying and receiving from overseas

If you live overseas and need a certificate, ask a friend/acquaintance/somebody of a laboratory you used to belong to in Japan to request it on your behalf to fill out the application form and submit it for you (paper form, not date).
Please discuss the return envelope and cost of stamps with your proxy.
The following two items are required in addition to the documents required for the application(Application form, Photocopy of your ID, Stamped, self-addressed envelope (with your postal code, name and address, a stamp)) .
-Letter of attorney: The individual names of the proxy and you are specified.
-Photocopy of Identification of the proxy (his/her passport/residence card etc.)
If you have no choice but to make a request from overseas by yourself, please enclose an “international reply coupon” and designate the mailing address.

*If a “designated form” (paper) has been specified by the organization to which the certificate is to be submitted, please enclose the designated form with the certificate.

*Other certificates include certificates of length of enrollment and certificates of withdrawal from school with satisfaction of credits and enrollment requirements. Please ask to us in advance if you would like to confirm the required certifications.

3.Envelope size and Postage charges

4.Number of days required for issuance (after an application form for the issuance of certificates is received)

[JPN] Transcript, Graduation: About 3 days after receiving the necessary documents
[ENG] Transcript, Graduation: About 1 Week after receiving the necessary documents
[J/E] Conferment of a Doctorate: About 1 Week after receiving the necessary documents
[J/E] Other certificates: About 1 Week after receiving the necessary documents
*The above guidelines do not include the number of days for mailing.
 Please note that we are unable to issue in a shorter period of time than this.

5.Notes

-No charge is needed for the issuance service.
    Pay postal fees only when mailing required.
-Applications for issuance of certificates are not accepted by phone, fax or e-mail.
-We do not issue certificates on Saturdays, Sundays, holidays, summer and new year’s holidays.
-We cannot pay postal fee temporarily on behalf of an applicant.
    Those who are living overseas and wish to apply for the issuance of certificates should have a
    Japanese representative to get Japanese stamps ready. (No cash-on-delivery is accepted.)
-Please note that we will not be able to confirm the arrival of the application documents or the
   delivery of the certificate. If you wish to know the delivery status, please use registered mail, etc.
